Your first visit

New patients are typically seen within about a week. Your first appointment is a real conversation, we review your history, your current medications, and your goals, then build a plan you understand before you leave.

What to bring

• Photo ID and your insurance card
• A list of current medications and dosages
• Allergies and relevant medical history
• Recent records or test results, if you have them

Filling these out before your visit saves time at check-in. Every new patient completes the demographics and insurance forms, then download the intake that matches your visit.

Insurance

We accept Medicare, Medicaid, and most commercial insurance, and will verify your benefits before your visit. We also welcome cash-pay (self-pay) patients, whether you are uninsured or prefer to pay directly.
